Description

How do we identify the particular healthcare needs of a child or young person with special educational needs

0 to 19 Torbay will offer your child regular developmental reviews and we work closely with our colleagues in other health care settings, nurseries and schools to provide practical help and Support.

We also run a number of groups in our Children's Centres to support your child with their development and socialisation skills.

We also have online Solihull short courses available to families and for young people, this can be accessed free of charge (Online Solihull courses the free access code is 'Tamar')

How do we consult with parents and/or children and young people about their needs?

We will meet with you, your child and your family to review and update the Family Health Needs Assessment.  We may use other assessments to get a clear picture of yours and your families need's.  this may be an opportunity to explore with you what help and support you may need.  It may be that we will discuss Early Help with you and talk about any additional support or resources that you and your family may find helpful.

How do we support children and young people with Special Educational Needs moving from children's services to adult service's?

The 0 to 19 Torbay service will work with young people up until the age of 19.  for those young people who need to move into adult services we will work with the family and young person to find the right resources to meet their individual needs.

How do we access and review progress towards agreed outcomes, and how are parent's, children and young people involved in this process?

The 0 to 19 Torbay service will communicate with families, children and young people in a variety of ways to suit their needs including British Sign Language and use of translators
